In Utah, kindergarteners and 7th graders have to be up to date on their immunizations before they can start school. If your child is not up to date, schedule an appointment now to beat the rush in August. Because of high demand, it is often hard to fit in last minute appointments.

To attend kindergarten, a student must have written proof of receiving the following immunizations:

  • 5 DTaP/DT
  • 4 Polio (3 doses, if 3rd dose was given on/after the 4th birthday)
  • 2 Measles, Mumps, Rubella
  • 3 Hepatitis B
  • 2 Hepatitis A
  • 1 Varicella (chickenpox) – history of disease is acceptable; a parent must sign the verification statement on the school immunization record.

To attend the 7th grade, a student must have written proof of receiving the following immunizations:

  • 5 DTaP/DT (4 doses of DTaP, if 4th dose given on/after the 4th birthday; 3 doses of Td, if starting series after age 7 years with a single dose of Tdap preferred as the first dose)
  • Tdap booster (Regardless of interval since the last tetanus/diphtheria containing vaccine)
  • 4 Polio (3 doses, if 3rd dose was given on/after the 4th birthday)
  • 2 Measles, Mumps, Rubella
  • 3 Hepatitis B
  • 2 Hepatitis A
  • 1 Varicella (chickenpox) – history of disease is acceptable; a parent must sign the verification statement on the school immunization record.
  • 1 Meningococcal
