Utah Valley Pediatrics is providing notice of a recent data security incident involving TriZetto (TPS), a third-party service provider that supports certain administrative functions for healthcare organizations nationwide.
What Happened
TriZetto recently informed us that it experienced a cybersecurity incident involving unauthorized access to certain systems within its network. According to TriZetto, on October 2, 2025, they became aware of suspicious activity within a web portal used to verify insurance eligibility verification. TPS launched an investigation after immediately terminating the bad actor’s access. TPS determined that access went as far back as November 2024. Upon notification, we promptly began working with TriZetto to understand the nature and scope of the event and to determine whether patient information associated with our practice may have been involved.
Based on the information provided to us, this incident occurred within TriZetto’s systems. Utah Valley Pediatrics’ internal systems or electronic medical record were NOT accessed or compromised.
What Information Was Involved
TriZetto has advised that the information potentially involved may have included certain patient demographic and/or billing-related information, such as:
- Name
- Address
- Date of birth
- Health insurance information
- Claim information
- Limited clinical information associated with billing
Not all information types were involved for every individual. The incident did not affect any payment card, bank account, or other financial information. At this time, neither we nor TPS is not aware of any identity theft or fraud related to the use of any affected individual’s information.
What We Are Doing
We take the privacy and security of patient information seriously. Upon learning of this issue, we:
- Promptly initiated a review of potentially affected records
- Coordinated with TriZetto regarding its investigation and remediation efforts
- Verified that appropriate containment and security measures were implemented
- Began providing notice to affected individuals, as required by law
- Reported the matter to appropriate regulatory authorities
TriZetto has indicated it has notified law enforcement and taken steps to further strengthen its cybersecurity protections. TPS is offering affected individuals complementary identity monitoring services including Single Credit Bureau Monitoring through a company called Kroll, including monitoring services for minors as applicable.

What You Can Do
At this time, we are not aware of any confirmed misuse of patient information as a result of this incident. As a precaution, individuals are encouraged to:
- Review explanations of benefits (EOBs) from health insurers
- Monitor account statements for unfamiliar activity
- Report any discrepancies to your insurer or healthcare provider
- Enroll in TPS provided credit monitoring services
Individuals are encouraged to remain vigilant against incidents of identity theft and fraud, review account statements, and monitor their free credit reports for suspicious activity and to detect errors.

Credit Reports. Under federal law, you are entitled to one free copy of your credit report every 12 months from each of the three nationwide credit reporting agencies. Fraud Alert. You may place a fraud alert in your file by calling one of the three nationwide credit reporting agencies above. A fraud alert tells creditors to follow certain procedures, including contacting you before they open any new accounts or change your existing accounts. For that reason, placing a fraud alert can protect you but also may delay you when you seek to obtain credit.
Place a Security Freeze on your Credit Report. You also have the right to place a security freeze on your credit report by contacting any of the credit bureaus listed above. A security freeze is intended to prevent credit, loans and services from being approved in your name without your consent. To place a security freeze on your credit report, you may be able to use an online process, an automated telephone line or a written request. The following information must be included when requesting a security freeze (note that if you are requesting a credit report for your spouse, this information must be provided for him/her as well): (1) full name, with middle initial and any suffixes; (2) Social Security number; (3) date of birth; (4) current address and any previous addresses for the past five years; and (5) any applicable incident report or complaint with a law enforcement agency or the Registry of Motor Vehicles. The request must also include a copy of a government-issued identification card and a copy of a recent utility bill or bank or insurance statement. You can place a security freeze and lift a security freeze on your credit report free of charge.
